Case managers play an important role in making sure seniors receive the care they need, no matter where they call home. They are the bridge between patients, families, and healthcare providers, coordinating services and resources to help clients live life to the fullest.
Each year during the second full week of October, the world recognizes case managers during National Case Management Week. It's a time to recognize the people who work behind the scenes to make sure seniors and adults with disabilities are well cared for. Lesley and Brandi took some time this week to show our appreciation to case managers across Polk County. They delivered small bouquets of fall-colored flowers, candy, and Starbucks coffee to show some love to the people who work so hard for our clients.
