How Does A Care Consult Work?

When you meet with our Care Manager - here's what you may experience:

Needs Assessment

The Care Manager will ask many questions to discover where your loved one could benefit from a helping hand, along with what aspects of daily life are going just fine right now.

Review of ADLs

Next, the Care Manager will ask questions about common Activities of Daily Living such as: Bathing, dressing, cooking, and transferring in and out of chair or bed. Are they still manageable, or do they require assistance?

Identify Opportunities for Support

Once the Care Manager has assessed the situation, they will explore care options the family may benefit from, based on any gaps or deficiencies noted.

Home Safety Check

A tour of the home will help identify any environmental safety concerns, such as trip hazards, lack of grab bars in wet areas, misuse of extension cords or space heaters – or any other safety concerns.

Social Assessment

Next, the Care Manager will identify any physical or other risks that may be associated with the aging process.

Care Coordination Plan

The Care Manager will then discuss any health concerns, medication reminders and other issues that should be addressed in a care coordination plan.

I want to thank Home Instead for taking such good care of our 88-year-old mother, and tell all of you readers about the very good experience we had in working with Home Instead. To put it mildly, my mother was a tough cookie. Set in her ways, independent, and didn't see that she needed help as she aged, didn't want "strangers" in her house. Her health declined rapidly and she also had dementia setting in. Home Instead set us up with 3 main ladies (with alternating shifts) who were so experienced they knew how to deal with mom, to the point that she looked forward to their visits. On 2 occasions, the door to my mother's house was locked when Home Instead arrived, and my mother had fallen inside and was on the ground. Not even knowing this yet, the Home Instead ladies didn't let a locked door prevent them from getting in to start their shift. They checked all doors and windows around the house until they got in, found her, then provided immediate help. That's just one of 100 examples I could make about how these ladies went above and beyond. Did they arrive on time? Yes, but they did so much more. My mother said the lunch they make her is "too pretty to eat." The ladies could get my mother to behave, eat on time, get ready for bed, etc., better than we daughters could! The excellent ladies that we grew to love are Michelle, Janet and Marjorie. Another important aspect of our relationship with Home Instead was the Office/billing/management responsibilities. The ladies in the office were very responsive and proactive. They auto-submitted our billing info to our insurance provider, which made things very easy for us. They kept in touch regularly, brought unplanned events or emergencies to our attention, and checked in regularly to see how WE felt things were going and asked for suggestions on how to improve. Bobbi and Shara were my main contacts there. Eventually my mother's condition became too difficult to handle at home and she is now living in an assisted living center. This is the only reason why we stopped working with Home Instead. If you're still reading this, you know how difficult it is to take care of aging parents and you're looking for help.
