How Does A Care Consultation Work?
When you meet with a Home Instead Client Care Coordinator here’s what you may experience:
Senior Needs Assessment

Needs Assessment

The Client Care Coordinator will ask many questions to discover where your loved one could benefit from a helping hand, along with what aspects of daily life are going just fine right now.

Review Senior Activities of Living

Review of ADLs

The Client Care Coordinator will ask questions about common Activities of Daily Living: Bathing, dressing, cooking, transferring in and out of chair or bed, those types of things. Are they still manageable, or do they require assistance?
Identify Senior Opportunities for Support

Identify Opportunities for Support

Once the Client Care Coordinator has assessed the situation, they will explore care options the family may benefit from, based on any gaps or deficiencies noted.
Home Safety Check

Home Safety Check

A tour of the home will help identify any environmental safety concerns, such as trip hazards, lack of grab bars in wet areas, misuse of extension cords or space heaters, or any other safety concerns.
Social Assessment

Social Assessment

The Client Care Coordinator will identify any physical or emotional risks associated with isolation and loneliness.
Senior Care Coordination Plan

Care Coordination Plan

We will then discuss any health concerns, medication management and other issues that should be addressed in a Care Coordination Plan.
Senior Care Consult Recommendations

Care Consult Recommendations

Finally, the Care Manager will review the results with you and make recommendations. Together, you can discuss the hours and types of care that would best support your family.
